Annual declaration of interests requirement : ‘Decision-making staff’
As part of the Trust’s commitment to transparency and in line with NHS England guidance on Managing Conflicts of Interest, all staff identified as decision-making are required to complete a Declaration of Interests form.
What is new

A new Declaration of Interests template form has been developed.
Guidance on the Declaration of Interests Form - This sets out the definition of ‘decision-making staff’ and descriptors of potential interests which must be declared.
If you are classed as a decision-making member of staff, you must complete the form at least annually or when circumstances change.
You are considered ‘decision-making staff’ if you are one of the following categories: Board member, Agenda for Change Band 8d or above, Consultant and SAS doctor.
What you need to do
When you have completed the Declaration of Interests form
- Submit it to ghn-tr.conflictsofinterestsghft@nhs.net
- Copy your line manager on the submission
If your line manager has not already signed the form, it will be returned for review.
Important Notes
- The form must include:
- All relevant interests
- Any gifts/hospitality over £50 received between April 2024 – March 2025
- All relevant interests or a nil return if you have no interests to declare.
- Declarations are also required:
- On appointment
- On any substantive change to the role
- Within 28 days of a new interest arising
- All interests, including null returns, are required to be collated into a Register of Interests, which will be published on the Trust website.
