- Support you with feelings, money, and health.
How to tell us what you need support with?
You will be offered a short form that helps us understand what matters to you. You can fill it out at home or with your nurse or support worker.
It helps us know:
- How you feel
- If you are worried, what you are worried about
- If you need help, what help you need
After the form:
- We talk with you about your answers
- We make a Care Plan just for you
- You get a copy, and we can send one to your GP if you want
