Background & Problem
- AKI associated with adverse short and long term outcomes for patient
- Community follow up required following AKI
- Sub-optimal information about inpatient AKI currently included on discharge summaries.
Aim
To improve the proportion of discharge summaries with the AKI section completed to 30% by April 2016.
Method
- PDSA Cycle 1 - Education

- PDSA Cycle 3 - Promotion campaign
- PDSA Cycle 4 - AKI drug chart sticker
Results
Percentage of summaries with AKI section completed has significantly increased since August 2015.
