In order for the laboratories to accept a sample for analysis, both the sample and request form must be adequately labelled.

Minimum data sets are described below. There are more stringent requirements for transfusion and blood grouping requests.

In general, inadequately labelled samples will not be processed and a repeat sample and request will be required.

When a sample is deemed to be unrepeatable there will be a discussion with the requesting clinician on an individual basis. Histology samples and request forms wil lbe returned to the sender.

Minimum data set for all samples except transfusion and blood grouping requests

  • Surname
  • Forename in full
  • Date of birth (not age) OR unique identity number such as MRN number or NHS number
  • If a unique indentification number is not included then the current residential address must be given on the request form
  • Printed labels are acceptable on specimens and request forms (stick a label on all request form copies)
  • Printed labels used on specimens (with the exception of Histopathology) should be signed or initialled
  • Data provided must be accurate
  • Request forms and specimens should be timed and dated

Minimum data set for transfusion and blood grouping requests

  • Surname
  • Forename in full
  • Date of birth (not age)
  • Unique identity number such as MRN number, major incident number or NHS number
  • For inpatients these should be checked against the patient ID band
  • Printed labels must NOT be used on specimens, but are recommended for request forms (stick a label on all request form copies)
  • All specimen labels must be handwritten at the patient's side by the person taking the sample as soon as the sample has been taken
  • The person taking the blood must sign and date the specimen and request form
  • The process must start and finish uninterrupted at the patient's side
  • Data provided must be accurate, matching on specimen label and request from
  • Specimens where patient data has been crossed out or altered will not be accepted
  • Use ballpoint as other pens may smudge
  • No specimen is considered unrepeatable

Unidentified patients

For unknown patients the following minimum data set is required:

  • Unique number, e.g. MI number
  • Approximate age
  • Gender
