Medical Photography Consent
Please read this consent information carefully. It will explain each consent level clearly. If you are unable to understand this information, please contact the Medical Photography department. A member of staff will be able to explain the consent levels and answer any questions that you may have. The telephone number is at the end of this page.
You will be asked to sign and date which level of consent you agree to on the Photographic Request and Consent form.
You do not have to agree to a clinical photograph or a video being taken. However, in some cases, refusal may stop your healthcare team from achieving the best possible outcome for you.
Level 1 - Health Records only
You provide consent to clinical photographs or videos being taken and stored as part of your health record. These clinical photographs or videos will only be used for the diagnosis and monitoring of your medical condition(s).
Level 2 - Teaching, research and clinical trials
You provide consent to clinical photographs or videos being taken and stored as part of your health record. These clinical photographs or videos will be used for the diagnosis and monitoring of your medical condition(s). The clinical photographs or videos will also be stored on the Trust’s teaching archive for staff to access. These clinical photographs or videos may be used for healthcare teaching.
Level 3 - Publication
You provide consent to clinical photographs or videos being taken and stored as part of your health record. These clinical photographs or videos will be used for the diagnosis and monitoring of your medical conditions. The clinical photographs or videos will also be stored on the Trust’s publication and teaching archives for staff to access.
These clinical photographs or videos may also be published in the public domain on websites and education platforms. It must be noted that these clinical photographs and videos may be viewed throughout the world and not just in the United Kingdom. Some overseas countries may not provide the same level of protection to the rights of individuals as EU/UK legislation provides.
Withdrawing consent
You have the right to withdraw consent for the use of your clinical photographs or videos at any time. Any request for consent withdrawal must be in writing and addressed to the clinical photography team. The email and postal address is at the end of this page.
Clinical photographs or videos consented for publication can only be removed from the Medical Photography system and cannot be recalled once published in the public domain.
Accessing copies of your images
Under Access to Health Records Act 1990 and The Data Protection Act 2018, copies of clinical photographs or videos can be provided. You can request copies of your clinical photographs or videos by contacting the Trust Access to Health Records department ghn-tr.accessoffice@nhs.net. The request will then be processed within 30 days, as per GDPR.
