Criteria Led Discharge (CLD) for GHT staff

Criteria-led discharge (CLD) is a process that empowers members of the multi-disciplinary team (MDT) including nurses, therapists etc to discharge patients who meet pre-agreed clinical criteria

The lead clinician should

  • identify the discharge criteria
  • complete the relevant documentation, the TTO and the discharge summary
  • select members of the MDT who can assess the discharge criteria

If there are no other concerns, a member of the MDT can discharge the patient once the criteria have been met.

This means patients get home more quickly because they do not need their clinical lead to approve discharge. The discharge process must then be completed on EPR.

Criteria Led Discharge (CLD) on Sunrise EPR

As we are waiting for a CLD box in EPR documents, please use the following

STEP 1

Click Add on the Weekend Registrar Discharge/ Review button

STEP 2

Enter further details into the Weekend registrar discharge/ review details box using the acronym expansion cld (which can be taken from the front page of acronym expansions) and enter the relevant information in the fields below

CRITERIA LED DISCHARGE (CLD):

The patient will be clinically ready for discharge if the following criteria are met. If there has been no change in their condition, they can be safely discharged with no further medical review required. (add criteria 1-5 or as required)

Designated discharge officer:

Nurse [ ] ACP/ANP [ ] Dr [ ] Therapy [ ]

If criteria are not met, or results are outside agreed parameters further medical review must be sought.

STEP 3

Nursing or Ward staff then need to note the patient is for CLD, and can use this to discharge from the ward over the weekend if CLD criteria are met.
