Primary Care Liaison service
The purpose of the Primary Care Liaison service is to provide a single point of contact to identify and resolve issues at the interface between the GHFT services and primary care teams, allowing a two-way dialogue to raise concerns and to identify and address themes.
Examples of categories for feedback are:
- Fit notes and inappropriate tasks: Requests for Fit Notes, investigation requests and action on non-incidental tests, blood tests that should have been completed by secondary care
- Onward referrals: Requests for primary care to enact onward referral for conditions related to the original referring complaint, instead of being handled directly by GHFT.
- Clinic Letters & Discharge Summaries: To clarify unclear responsibility for follow-up actions, poor formatting or ambiguous communication
- Follow-up Clarity: Queries about follow-up arrangements or whether a patient is open to a GHFT team. If the patient is known to be open to a team, but might need earlier follow-up, please direct your points to the relevant clinical team.
- Medication Shared Care: Inappropriate requests for primary care to prescribe medications which should have been prescribed in secondary care.
This service is not for:
- Clinical queries or patient management decisions
- Requests to expedite referrals or planned follow-up due to a change in clinical status - this should be directed to the responsible clinical team.
Please note that GHFT Primary Care Liaison Service aims to respond within 10 working days. If your feedback relates to a more urgent clinical issue, the GP is to maintain responsibility for taking appropriate clinical actions. Submission of feedback does not constitute a transfer of care from the GP.
